About the Role:
As a Care Coordinator, you will play a crucial role in service linkage and care coordination for children, youth, and families with significant behavioral health needs. Your work will involve engaging with families, understanding their unique needs, and ensuring they have a voice in their care. You will be an essential part of a team that values strong community partnerships and supports families in navigating resources effectively.
Eligible Counties:
To apply, you must reside in one of the following Ohio counties: Coshocton, Fairfield, Guernsey, Morgan, Muskingum, Noble, Perry, Washington, Athens, Fayette, Gallia, Jackson, Highland, Hocking, Meigs, Pickaway, Pike, Ross, or Vinton. This position will require quarterly in-person meetings in various CME counties.
Key Responsibilities:
Serve as a liaison care coordinator for designated regions and counties.
Assist with care coordination in counties needing immediate or temporary support.
Collaborate with families to identify service needs in alignment with program goals for positive outcomes.
Understand cultural factors influencing family dynamics to enhance engagement and success in care planning.
Identify the strengths of children and families to support healthy outcomes.
Coordinate family-based services in home, school, and community settings.
Ensure care plans are appropriate, least restrictive, and address safety and treatment needs.
Foster positive relationships with children and families to integrate care plans effectively.
Develop creative partnerships with community resources to meet diverse needs.
Maintain necessary documentation and participate in evaluations and team meetings.
Stay current with training requirements, including High Fidelity Wraparound and Cultural Humility.
Perform additional duties as assigned.
Minimum Qualifications:
Experience providing services to children and families in behavioral health or related fields:
Three years with a high school diploma or equivalent; or
Two years with an associate degree or bachelor's degree; or
One year with a master's degree or higher.
Lived experience is encouraged.
Knowledge of Hi-Fidelity Wraparound preferred (Certification provided at employment).
Two years of experience in a supportive services or care coordination role preferred.
Experience working with individuals with autism spectrum disorders or developmental disabilities preferred.
Familiarity with areas such as family systems, community resources, case management, and child development.
Culturally humble with training and experience to manage complex cases.
Strong organizational skills with the ability to prioritize tasks effectively.
Excellent communication skills, both verbal and written.
Ability to operate in an internet-based, automated office environment with reliable internet access.
A valid driver's license is required.
